What a cholesterol ratio actually tells you

A lipid panel hands you four raw numbers and no context. Total cholesterol counts everything your blood is carrying, including the HDL fraction that ferries cholesterol back to the liver for disposal. That is the awkward part: a chunk of your "total" is the helpful kind. Ratios fix the accounting by dividing the whole burden by the protective share, so one figure carries both halves of the story. Every ratio on this page follows the same rule — lower is better.

Worked example: total 190, HDL 55, LDL 110, triglycerides 120 mg/dL

Start with the headline ratio. Total cholesterol divided by HDL is 190 / 55 = 3.45, which the tool displays as 3.5 and bands as average — the ideal range stops just below it, so this panel sits right on the line. Next, LDL divided by HDL is 110 / 55 = 2.0, comfortably inside the good band under 2.5. Non-HDL cholesterol is simple subtraction: 190 − 55 = 135 mg/dL, a shade above the widely used general target of 130. Finally the triglyceride/HDL ratio is 120 / 55 = 2.2, just past the 2.0 mark often quoted in insulin-sensitivity research.

Read together, that is an unremarkable panel with one soft flag. Nothing screams emergency, but non-HDL is nudging the target and the triglyceride ratio suggests carbohydrate load is worth a look. Now change one input: keep everything else and drop HDL to 35. The TC/HDL ratio jumps to 5.4 (elevated), LDL/HDL becomes 3.1 (borderline), non-HDL rises to 155, and triglyceride/HDL hits 3.4. Same total cholesterol, a completely different picture — which is exactly why the single number on the top line of your report is a poor summary on its own.

If your report is in mmol/L, switch the unit selector and the tool converts internally at 38.67 for cholesterol and 88.57 for triglycerides before applying these cutoffs. That conversion matters most for the triglyceride ratio, whose thresholds were derived from mg/dL data and are meaningless if applied directly to mmol/L figures — a common mistake in UK forums.

Ratios versus absolute numbers: an honest history

Through the 1990s and 2000s the TC/HDL ratio was a headline metric in cardiovascular risk discussion, partly because the Framingham risk equations used it directly. Guidelines have since moved on. The 2018 ACC/AHA cholesterol guideline in the US and NICE guidance in the UK now lead with absolute LDL cholesterol and non-HDL cholesterol, plugged into a whole-person risk calculator — ASCVD in the States, QRISK3 in Britain — that also weighs age, sex, blood pressure, smoking status, diabetes and family history.

Why the shift? Because therapy targets a quantity, not a proportion. Statins, ezetimibe and PCSK9 inhibitors lower LDL by a measurable amount, and trial benefit tracks the absolute drop in LDL particles. A ratio can improve while your LDL stays exactly where it was, simply because HDL rose — and the large HDL-raising drug trials failed to produce the heart-attack reduction that observational data predicted. That is the honest caveat: high HDL is a marker of lower risk, but raising it pharmacologically has not proven to be a lever.

Ratios still earn their place as context. They are quick, they resist the panic that a lone total cholesterol number causes, and non-HDL in particular is now a formal secondary target in both US and UK guidance because it captures every atherogenic particle in one subtraction.

Levers that move the numbers, ranked by evidence

Strongest first: eliminate industrial trans fats, which raise LDL and lower HDL simultaneously. Replace saturated fat with unsaturated fat rather than with refined starch — swapping butter for olive oil moves LDL, swapping butter for white bread mostly moves triglycerides upward. Cut refined carbohydrate and alcohol if your triglyceride ratio is high, since both feed hepatic triglyceride production. Add 150 minutes a week of moderate aerobic activity, which lifts HDL modestly and lowers triglycerides more. Lose excess weight; roughly every 10 lb lost is worth a few points of LDL. Stop smoking — cessation alone can raise HDL by around 5 mg/dL within a year. Soluble fibre from oats, beans and psyllium contributes a further small LDL reduction.

When to see a doctor

Book an appointment if your TC/HDL ratio is above 5, your non-HDL is above 190 mg/dL, your triglycerides exceed 500 mg/dL (a pancreatitis concern rather than a heart concern), or you have a family history of heart attack before 55 in men and 65 in women, which can point to familial hypercholesterolaemia. Bring the whole panel, not the ratios.

Statins and other medicines move LDL far more than lifestyle does, and that decision belongs to your doctor.

Do I need to fast before the blood test?

For triglycerides, classically yes — a 9 to 12 hour fast. Non-fasting panels are now accepted for routine screening in both US and UK practice because total, HDL and non-HDL barely shift, but triglycerides can read noticeably higher after a meal. If your triglyceride/HDL ratio looks high on a non-fasting sample, a fasting repeat is the usual next step.
